Electronic Funds Transfer Authorization Assessments (U.S.)

This form authorizes the Christian Reformed Church (CRC) to automatically withdraw monthly Ministers’ Pension Plan (MPP) assessment payments from a designated bank account. It collects basic contributor information, banking details, and a signature granting permission for electronic funds transfers. By signing, the church or participant agrees that the CRC can debit the specified account until written notice of cancellation is provided. The form also includes instructions for revoking the authorization and contact information for the CRC Ministers’ Pension Plan office for any questions.
