Evaluating pastors and ministry staff is a fairly common occurrence within churches, but how often are ministry programs evaluated? How do churches assess how effectively their programs have remained focused on the mission of the church and that volunteers are being equipped and empowered to do their tasks? Attached is a Ministry Evaluation form tool that was developed by one church to assist in evaluating their ministry programs by gathering feedback from volunteer ministry leaders. Feel free to edit it for your church’s purposes.

I was the Ministry Coordinator at CrossRoads when this was developed, and while I don't remember specifics of the results, I thought it was a great tool that ended up providing really helpful feedback. I'm glad to see that it's being shared for others.
