Request Forms for Benevolence?

I'm a fairly new deacon at my church and we're looking to make revisions in some of the forms we use for benevolence requests. I'm wondering if churches document requests for financial assistance from their members. We want to keep track of money allocated to those in need but feel it's not necessary to ask for extensive information from them. How does your church keep records? What information do you require? Any suggestions would be greatly appreciated.
