Hello, We will be conducting performance reviews for paid staff at our church. We have a form for that process but we'd also like to give congregation members the opportunity to submit feedback about the pastor via a survey. Do you have any sample surveys for this purpose? Thanks!

Rob, our office (Pastor Church Resources) has created a training tool on pastor evaluations. You can access a copy here https://network.crcna.org/sites/default/files/documents/Evaluation_Essentials_Congregational_Leaders.pdf or you can order free hard copies from Faith Alive 800-333-8300 M-F, 8:00-5:00 EST (the only cost is for shipping and handling).
