CRCNA endorsed chaplains are strongly encouraged to provide an annual report on their ministry, which is shared with the calling church and Thrive/Chaplaincy Ministry. This serves to connect chaplains with their churches and the denomination, and enables the church to better support and participate in chaplains' ministry. Annual reports can be sent to Thrive/Chaplaincy Ministry at [email protected]. 

Chaplains may use this form to write their annual reports.
